Check your documents before going to the DFA to get a Philippine Passport
We’re planning to have our first ever out of the country trip this year. I already bought our tickets last January even though we don’t have our passports yet haha. After our visit at DFA-Alabang last April 1, I was thankful that our travel period is on August and not this month of May which is the original plan.
Why? because we’ve got a little problem with our documents. I decided to post this because I don’t want you to experience the same thing. I don’t want you to go back again and again because of problems in your documents. I am concerned especially if you’re coming from the provinces.
I. FOR FIRST TIME PASSPORT APPLICANTS:
• Personal appearance
• Confirmed appointment
• Duly accomplished DFA Passport Application form– may be downloaded from the DFA website, www.dfa.gov.ph
• Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA. Transcribed Birth Certificate from the LCR is required when entries in PSA Birth Certificate are blurred or unreadable. (REPORT OF BIRTH DULY AUTHENTICATED BY PSA IF BORN ABROAD)
• Valid picture IDs and supporting documents to prove identity (Please refer to List of Acceptable IDs and List of Supporting Documents)
The following applicants need to submit additional requirements:
- Married women — marriage contract issued by the NSO.
- Senior Citizen with Senior ID
- Person with Disability (PWD) with PWD ID or visible disability
- Solo Parent with Valid Single/Solo Parent ID
- Pregnant women with medical certificate
- Minors seven (7) years old and below
Noel, got his passport in less than 30 minutes because there was no problem with his documents unlike with me and Ren. That was fast yes, I commend DFA Alabang for that. I guess Friday is the perfect day to schedule your DFA appearance.
As per the the documents needed posted on the DFA website, I brought with me an original NSO copy of my birth certificate and marriage contract issued by the Local Civil Registrar and duly authenticated by NSO.
But they told me that they wont accept it because they want a latest NSO copy.
I didn’t know that there’s an expiration date with government documents. I don’t get the logic why they need the latest copy. The information in it is still the same even if I get a new one, right? hmmmm….
But, I can’t do anything but just to follow.
On the other hand, Ren’s NSO birth certificate problem is that the “Place of Birth” area is empty.
I have a copy from the Local Civil Registrar and as you can see the place of birth is visible here.
Still, the DFA didn’t want to accept it. They asked me to go to the Municipality where Ren was born and file for a supplemental report.
That’s why it’s important to check your NSO documents first. If you’ve noticed some discrepancies like missing letters, wrong spelling, wrong date, without place of birth etc. then it’s better to visit your municipality and file a supplemental report before going to the DFA.
Because of my busy schedule, it’s only last April 25 when I had the chance to visit Manila City Hall.
Here are the requirements for a supplemental report.
I thought I won’t be needing other local documents because I already have a SECPA copy of Ren’s birth certificate, but they still asked me to get a latest copy. Again, I don’t get it. Why does everybody wants a latest copy when the information they’ll be getting is still the same???
Overall I spent P650 for this supplemental report that in the first place is not my fault. It also took me 2 hours to finish all the procedures needed to get the Certified True Copy of Ren’s birth certificate.
Transmittal to NSO: P190.00
Certified True Copy of Birth Certificate: P50.00
Notary Public : P100.00
And worse, I need to return the next day to get it. Imagine the time, effort and cost I spent for this. Argh!!!
After finishing the supplemental report (which I think took me 2 weeks), I brought the documents to NSO/PSA office in Quezon City. After filling up some information there, they will ask you to go back after a week to get the corrected copy.
Overall, it took me almost a month to finish the process.
