UPDATED 2018: Check your documents before going to the DFA to get a Philippine Passport
Yes guys it’s important to check your documents first before going to DFA to get a Philippine Passport. I don’t want you to experience the same thing that we have experienced. It took me a month to finish the whole process of correcting my son’s PSA birth certificate.
I. FOR FIRST TIME PASSPORT APPLICANTS:
GENERAL REQUIREMENTS
• Personal appearance
• Confirmed appointment
• Duly accomplished DFA Passport Application form– may be downloaded from the DFA website, www.dfa.gov.ph
• Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA. Transcribed Birth Certificate from the LCR is required when entries in PSA Birth Certificate are blurred or unreadable. (REPORT OF BIRTH DULY AUTHENTICATED BY PSA IF BORN ABROAD)
• Valid picture IDs and supporting documents to prove identity (Please refer to List of Acceptable IDs and List of Supporting Documents)
The following applicants need to submit additional requirements:
- Married women — marriage contract issued by the NSO.
- Senior Citizen with Senior ID
- Person with Disability (PWD) with PWD ID or visible disability
- Solo Parent with Valid Single/Solo Parent ID
- Pregnant women with medical certificate
- Minors seven (7) years old and below
OUR STORY:
Getting a passport can be as fast as 30minutes if you will not encounter any problem in your documents. We went there on a Friday afternoon and the process was smooth. My husband did not encounter any problems with his papers, unlike with me and my son.
As per the the documents needed posted on the DFA website, I brought with me an original NSO copy of my birth certificate and marriage contract issued by the Local Civil Registrar and duly authenticated by NSO.
But they told me that they won’t accept it because they want a latest NSO copy.
I got this November 2007
I didn’t know that there’s an expiration date with government documents. I don’t get the logic why they need the latest copy. The information in it is still the same even if I get a new one, right? hmmmm….
But, I can’t do anything but just to follow.
On the other hand, Ren’s NSO birth certificate problem is that the “Place of Birth” area is empty.

I have a copy from the Local Civil Registrar and as you can see the place of birth is visible here.

Still, the DFA didn’t want to accept it. They asked me to go to the Municipality where Ren was born and file for a supplemental report.
That’s why it’s important to check your NSO documents first. If you’ve noticed some discrepancies like missing letters, wrong spelling, wrong date, without place of birth etc. then it’s better to visit your municipality and file a supplemental report before going to the DFA.
Because of my busy schedule, it’s only last April 25 when I had the chance to visit Manila City Hall.
Here are the requirements for a supplemental report.
I thought I won’t be needing other local documents because I already have a SECPA copy of Ren’s birth certificate, but they still asked me to get a latest copy. Again, I don’t get it. Why does everybody wants a latest copy when the information they’ll be getting is still the same???
Overall I spent P650 for this supplemental report that in the first place is not my fault. It also took me 2 hours to finish all the procedures needed to get the Certified True Copy of Ren’s birth certificate.
Registration: P310.00
Transmittal to NSO: P190.00
Certified True Copy of Birth Certificate: P50.00
Notary Public : P100.00
And worse, I need to return the next day to get it. Imagine the time, effort and cost I spent for this. Argh!!!
After finishing the supplemental report (which I think took me 2 weeks), I brought the documents to NSO/PSA office in Quezon City. After filling up some information there, they will ask you to go back after a week to get the corrected copy.
Overall, it took me almost a month to finish the process.

Hi Ma’am! Same din po yung case ko tulad nung nangyari sa inyo, last friday i had an appointment at DFA and noon ko lang din po napansin na blank yung place of birth ko sa PSA ko. Talaga po bang ganon katagal yung proccessing time kase binigyan lang ako ng one month para ayusin yung sa PSA ko, kung hindi ko maayos within a month, need ko na naman po magpa-appoint ng new sched sa DFA 🙁

Mommy Levy. Ano po ang itsura ng transmittal slip? Yun daw po kasi ang hinahanap ng DFA para katibayan na nagpaayos daw talaga sa PSA.

Mam paki help nman po,. My mom is born and raise in Cebu City. Ang parents nya po chinese. Ang birth certificate nya po nka lagay chinese citizen. Papaano po ito ma correct para po mka kuha ng passport ang mama ko. Senior citizen na po ang mama ko. Thank u
sorry I can’t help you since I don’t know the process on her case. My advice is to visit the municipality where she was born and ask them what to do. Pwede nyo po ipagawa siguro sa mga kamag-anak na andon.

as far as I know yes. Pwede mo naman ipakuha yun. Ask mo din sa local registrar saan ka malapit para sure, then balikan mo ako kung pwede hindi kung saan lugar ka pinanganak kumuha.

Nag appointment ako dati sa DFA Aseana only to have the tell me dapat same yung birth place sa birth certificate and sa Appointment form. Mali kasi nung hospital, nilagay lng sa birth place ko is “QMMC” which is the shortned name of the hospital, pero nilagay ko sa Appointment form is complete like “QMMC, Quezon City”.
So better have it corrected or at least kung ano nasa birth certificate mo, same din lagay mo sa forms.

Hello po goodeve po. Acceptable po ba ang employment id, philhealth id, brgy id? Yan lang po kase meron akong id’s. Pero meron po ako supporting documents like TOR, Diploma and NBI clearance? Ma approve po kaya ang pag process ng appointment ko?? Thankyou po sa pag sagot. Godbless po!

Hello po. Last Oct 16,2018 galing po ako sa DFA Calasiao. Nag kaproblema po ako sa BC ko kelangan po ipa annotate. Eh 4-6 months po ang process nun. pinababalik po ako ngaun buong month ng november.Di naman na po ako makabalik at kulang po hawak ko papel. Pano po kaya ung binayad ko?

I actually don’t know. Sabi ng iba tinatanggap, sabi ng iba hindi. Ang advice ko kung may time ka pa, kumuha ka na lang ng PSA copy para sure.
Regarding the copy of Birth Certificate from NSO, it doesn’t have to be the latest copy as long as it is from the NSO since there is already a directive from the NSO that BC from NSO DOES NOT expire.

wish i saw your blog sooner. had my appointment earlier in DFA-Alabang ang they refuse to honor the NSO issued w/ OR attached BC. Copy was back in 2004, stating it’s too old and they can only honor the yellow sealed NSO paper. As if my BC will change!!! Such waste of time!

Correctional na daw po iyun. Wag daw po maniwala s dfa na supplemental lang. And it will take 4 to 5 mos pa. BUT in case nung skin yung name ng hospital ay nakagitna na sa place of birth. Correctional n daw yun, kung supplemental yun maitatapat na s column ng provice. D nman daw province ang Manila. KAYA my chance pa na supplemental lang kung my space pa para sa idadagdag na info.

Pahirapan kasi appointment ngayun, madalali maubos ang mga slots sa website nila.
as far as I know, valid for 1 month yung schedule mo and pwede ka bumalik anytime within that period.
The problem is that 1 month lang yung extension na binibigay nila pero the process for the supplemental (in this case name ng hospital only) takes almost a month. So by that time expired na schedule mo with DFA. What an inefficient Philippine government system. All the more I am beginning to be disappointed with this country.

I had the same experience,,nawala passport ko this year so i decided to request for a passport replacement,kumuha ako ng Affidavit of loss, Police report and sched an appointment with the DFA along with my valid ID’s and PSA birth cert,but unfortunately hindi nila tinanggap yung PSA birth cert ko need ko daw kumuha ng local copy of my birth cert which is i don’t have an idea why I need to submit such document wherein my copy naman ako ng NSO birth and PSA birth cert. I don’t know what’s the basis of asking me to submit the said documents malinaw naman yung copy ng birth cert ko.
minsan parang sinasadya lang ng mga government personnel na patagalin ang process or minsan sinasadya lang nila manghinge ng mga documents para rin kumita sila.

Magchange status po sana ako sa passport single to married 2021 pa yong expiration ng passport ko kaso yong hawak ko na ID is TIN id lang expired na kasi yong iba kung id okay lang ba yong TIN id? Yong passport ba pwedi na pinakaprimary?
I think pwede na yung old passport mo then PSA copy ng married contract. Or pwede din kumuha ka na lang ng Postal ID with your new surname.

perhaps you can ask them to edit it na lang. Walang option online to edit it kasi kaya sa dulo pinapa check muna lahat ng na input bago iconfirm.

Hello momshie. Ask ko lng sana if may idea kayo sa concern ko. Last yr kasi di ako nakapagpatuloy magrenew ng passport ggamitin ko sana ung surname ni husband kasi nga kasal na ako pero nakitaan ng mali sa place of birth ng Marriage Contract namin. Need ko magfile for correction. Pero hndi ko onasikaso noon kasi nasadden po ako sa nangyari naisip ko na hndi tlaga para sa akin magabroad. Pero ngayon i have limited time nalang para kumuha ng passport ko kasi kailangan na raw talaga na makaalis ako need ko po mapadala by december to spain yon. Alam ko pag nagfile ako di na aabot kc 3-6 mos bago macorrect ung mistake. Dami pang need na requirements and shempre fees na naman.
Pinaplano kong gamitin ang surname ng pagkadalaga ko para makarenew pero need ilagay sa appointment married pa din po ako dba? Nabasa ko may rights naman daw ang married woman na gmitin ang maiden name ko pa rin. Kaso iniisip ko baka ipadala na naman ang mali sa Marriage contract ko. Plus id ko surname na ng asawa ko. yes hindi natin kasalanan yung mga mali nila, pero still tayo ang dapat nag aasikaso. Sayang yung bakanteng panahon na sana naayos mo na. In this case, it’s up to you kung anong surname ang gagamitin mo. Regarding your ID siguro pwede ka magpa postal ID na lang using your surname and not your husband.

ask ko lang po kung ipoprocess po ba ng DFA yung renewal ng passport ng husband ko kasi hindi nya po naiattached yung birthcertificate nya and xerox ng valid id. Hindi daw kasi hiningi at hindi nya na rin ininsist na ibigay.. paano po yun? mapprocess po ba yun? Yesterday lang po yun at half month pa bago ang releasing..

If in case na di tanggapin ng DFA ang BC nyo, it’s simply means na may maling info sa BC nyo or bka dhil malabo na.
So always check your BC prior your appointment pra macorrect agad and avoid delay in processing your passport application/renewal.

mali yung nainput mo o mali yung mismong copy ng Birth Cert? Kung yung mismong copy ng birth certificate, I think supplemental report ang kailangan para ma correct yun. Kung yung ininput mo lang naman ang mali, pwede mo na lang ipa correct sa DFA mismo.

I got my NSO birth certificate year 2009, malinaw pa din naman sya at nababasa pa yung mga informations. Tatanggapin pa din po ba yun ng DFA or I have to get pa the latest one, which is psa na ngayon? Thank you po 🙂
get a new one if you still have the time to do so. They might or might not ask for a PSA copy, mas okay na yung ready.

you need to fix your birth certificate from your municipality. There is no alternative document for birth certificate I think.
Base on my experience… Nag process ako ng passport ko there is no checck sa gender. Pumayag nmn sila ng notar or bring supporting dox. Kpag may anak ka pwd birthcert ng baby mo as supporting dox katibayan na babae ka. But that was long time ago 2009. D ko sure kung ganun prin ngaun.

Nakapag book po ako ng appointment kahapon sa dfa para sa passport and hindi ko inaasahan monday next week na makuha ko, grab ko na din since super hirap mag hanap ng schedule. So i decided to get a new birth cert which is psa na, kaso nagka problem wala po akong gender sa birth, so i need to go back sa munisipyo kung saan ako pinanganak, pag punta ko dun ang daming requirements, like id’s katunayan na female ako., medical records at affidavit or supplementary. Nabigay ko namn po lahat binagay din saken ung copy of live birth ko ang kaso po sabi ung endorsement letter ko na ipapasa sa dfa after 15 days pa daw po. Possible po ba matanggap itong live birth ko kasama ng ibang documents ko. Hay tapos yung name ko po sa appointment online kulang ng joy. Possible po ba maayos ito duon 🙁 thanks you.

Ang alam ko po, kailangan ng LCR copy, at meron din po ako nun kaso yung LCR ko ay 2001 pa na issue. Di rin po kaya ito tatanggapin? support lang naman po ito dun sa PSA ko. kung hindi po ito tatanggapin, pwede po bang ibang docs na lang ang gamitin at hindi LCR? salamat po.

The PSA copy I ordered online is yet to be delivered, I’m wondering if it’s alright if I follow up the BC na lang since malapit lang naman workplace ko. I just finished my appointment in ASEANA and fortunately they never asked for my birth certificate. I simply handed out the filled out form, 2 copies of the e receipt, and a photocopy of my passport. Then dere derecho na yung processing. Not sure lang kung ganito din sa ibang branch but thankfully hindi humingi ng cooy ng certificate ko.

Are these requirements applicable sa lahat nang DFA offices? I just submitted kasi lahat nang pwede masubmit except yung acceptable valid ID nila. I had PSA, police, NBI, barangay clearances and I photocopied pa ang tin Id, philhealth and company id pa.. The travel agency said na they’ll try to submit my papers pero wala assurance kasi nga I don’t have valid ID.. Haay

I don’t know if it’s important since details mo ang importante sa kanila (not your parents name). But still di ko masasagot ng tama ang tanong mo dahil di naman ako empleyado ng DFA.

Nakakuha na ko ng passport ko. I am asking for my boyfriend’s behalf. Last 2017 nagsabay po kami pumunta para kumuha passport kaso di naprocess un kanya kasi malabo yun birthplace niya sa NSO niya. Ngayon nagpasched ulit siya, this coming June 22 na po. Meron siyang 3 original copy ng Live Birth Cert niya na medyo kita na un birthplace and supplemental report made by an attorney and un NSO niya pa din po dati. Iaaccept po kayao yun? Matagal kasi maiayos un malabong place of birth niya sa Manila 🙁

Ayun SA site nila marriage certificate from local registry office and duly authenticated by PSA kung ito po dala ko tatanggapin po kaya nila? Kung Hindi, masasayang po ba ang appointment ko kung lack of documents ako? Need ko po ba magpaappointment ulit sobrang hirap kasi kumuha ng slot. we have the same problem. You need to file petition for correction of clerical error of your birth certiifcate sa LCR. I’ve been waiting for 10 months already. And it was not yet corrected. Hindi rin kasi ako kinontact ng LCR na I have to pay some fees after they received the decision from OCRG. YUP. Ganun Kabagal.

Hi good day! Last September 2017 kasi nag apply ako for passport but na deny kasi may mali sa BC ko so tinatakan po nila yung form ko and write remarks na need ko ng new corrected BC. After months now lang na release ung corrected BC ko. Need ko po kaya mag pa appointment ulit or valid pa naman ung form ko dati? Thank you.

usually may email galing sa DFA mismo kung successful ang appointment nyo. Pwede mo irequest na email mo ang gamitin sa pagpapa book para direct na matanggap mo from DFA ang confirmation

Hello po. Magshare lang po ako. Kumuha kame kanina ng passport sa DFA-ASEANA and ang dala ko lang na requirements ay NSO (not PSA) at voter’s id, yun lang. Ang dali ng naprocess ng passport application ko. Mabilis ang pagkuha ng passport sa DFA-ASEANA. yun lang po. :))

Hello po. Ask lng po ako, last January I went to DFA Tacloban to apply for a passport for me and my daughter. My daughter’s requirement is all good, she is 2y.o by the way. Tung sakin is yung my issue. I brought with me the original copy of my NSO. Ayaw nilang tanggapin because they said my middle name has blotches, I was born 1991 so yeah typewriter pa gamit dun. Although readable nmn yung middle name ko but they won’t accept it. So at the time they suggested something to me about going to the LCR where I was registered. After that, di ko na maalala yung sinabi nila kase nga shookt pa ako at the time. Imagine the long travel tapos wala palang maachieve. So ask ko lng po ano po gagawin ko when I go to the LCR, taga Leyte po ako, tapos Cebu ako pinanganak. Supplemental report din po ba? Or what? Sana po maliwanagan ako. Plan ko pumunta nang Cebu by May. Thank you po
yes I think supplemental report po ang kukunin nyo. KUng may kilala kayo sa Cebu pwede nyo po siguro pakiusapan na sila mag asikaso.

Kumuha na po ako ng Certified True Copy ng birth cert, niya sa Local Civil Registrar sa amin sa Bacoor City. Ang hindi ko po maintindihan ay kailangan pa daw ito ng authentication ng PSA. Hello! I also had a problem with my birth cert on my first name. I filed a petition last Nov, 2017 here in my municipality. And it was affirmed by the CRG and MCR received it on Mar21, 2018. The finality was sent to PSA-Manila last Mar26 to have it online. My schedule for appointment for passport Apr6, 2018. Fortunately, the accepted my application for passport even if there’s no annotated BC. I only gave them a copy of my petition and a copy of an action taken from the CRG that is affirmed and I prayed that it be accepted. God is good! Hope this information can help.

If all the requirements you have mentioned are included in the list indicated at DFA website then there will be no problem in getting a passport.
Hi, I have the same issue with my birth certificate na missing yung place of birth. We tried what you did pero iba ang sinabi samin ng Caloocan city hall na hindi na sila nag iissue ng supplemental report. Ang finollow namin na process ay yung correction of clerical error na almost 3 months ang processing. Ganun na po ba tlga? Ang sinabi sakin ng DFA nun ay kumuha lang din ng supplemental report. Hi! We’re done with our appointment in DFA Alabang. Just want to give an update to everyone here. I was able to finally get my UMID. Yun lang tinanggap na ID ko ng DFA employee. Sabi niya hindi daw need yung premium postal and tin card ko. Hindi na naghanap ng supporting documents. I was just asked for my and my daughter’s original birth certificates, photocopies of our certificates and IDs. Same with my husband. Yung concern ko about his birth certificate na abbreviated yung place of birth: Cab. City NE = hindi na pinansin ng employee na nagcheck ng documents niya. Thank God. He provided his UMID and driver’s license.
May photocopier sa DFA Alabang branch but I suggest that you make 3 copies for each of your documents/IDs so you can breeze through the lines. Our appointment was at 10am. We got there at 8am. May mga nakapila na. The good thing is we were able to finish our application in just 30 minutes.
Tinanggap yung PSA birth certificates that we got last year. May nakasabay kaming guy kanina na pinabalik kasi blurry daw copy ng birth certificate niya.

Hi! Baka po may nakakaalam sa inyo. Ask ko lang po if tatanggapin po ba ng DFA kpag yung last name ko UMID ID surname ng father ko, and ung nsa BC maiden name ng mama ko sa pagdalaga.. Lahat ng documents ko since bininyagan ako last name na ng father ko ang gamit pero ngyon ko n lng po gingamit ung apelido ni mama nung dalaga p sya. Un lng kasi na primary ID ang kya kong i provide. “Unknown” din pla sa BC ko si papa kya sobrang laki ng prob ko tapos 2003 lng sila naikasal and 2005 namatay si papa. 1993 po ako pinanganak.. Sana po may sumagot kasi need ko po tlga ng passport may nNgyari kasi sa fiancé ko.

Maam Levy gud eve po. Ngayung araw lang po ako natapos sa pag.process nang passport ko. Regarding po sa mga tanong nang lahat kagaya ko din…! Kung valid paba yung (BC) na may logo nah NSO. Dito sa dfa cebu opo. valid pa po. midyu na delay lang po ako kunti kanina sa processing area kasi pinakuha po ako nag LBC(local birth cert.) dito sa city health namin. Nah may CTC(certified true copy)Kasi po daw may letra nah hindi klaro… yun dalidali akong nagpunta sa city health namin dito para naka kuha nang BC. Para maka balik agad sa dfa. Sa awa ni GOD natapos din. Midyu sayang nga lang kasi kinuha nila yung original nah BC na may NSO logo. At yung original nah LBC(local birth cert.) Copy ko. Sana mapansin nyo po itong post ko. May mali po kasi sa birth certificate ko which is isang letra sa middle name ko. Pero nakapagfile na ko ng petition for correction of my middle name sa BC ko this February. Binigyan na rin po ako ng certification from LCR na nagpapatunay na in-process na yung correction ng BC ko. 6 months po kasi yung processing nung correction.

If you’re near the DFA office, I think you can ask yung nasa information desk regarding your concern. I went to DFA Dgte yesterday and pinapapasok naman po nila yung mag.iinquire regarding the requirements paisaisa. You can call the office where you set your appointment din.

Ang ginawa daw ng iba ay nagdala ng old school IDs, transcript of records, old employment IDs, ITR, yearbook, baptismal certificate aside from the NBI, police clearance. I suggest na kumalap kayo ng mga old IDs para sure. May hinanapan nga daw ng old growing up pics eh. Siguro gusto talaga manigurado ng DFA employee. Haha! Nagprep din po ako ng documents ko for our appointment next week. I’m just waiting for my UMID to be delivered. So far, I have the premium postal ID I got last year pa, LTO student permit I got last month, TIN card, TOR from my university, old college ID, old employment IDs, PSA certificates, baptismal certificate (yung friend ko hiningian nito), SSS E-1 form. I’ll also bring my yearbook, old Philhealth MDR. I’ll also bring yung certified copy ng birth cert ko from the LCR and yung request for correction sa first name ko. Kahit madaming bitbit papunta dun, mabuti na kaysa pabalikin. For my daughter, I’ll bring her old school IDs, birth certificate, yung certificate of birth from the hospital and form 137.

My wife and my 2-year old son just finished their passport application last Friday, Feb. 23, 2018. My wife’s Birth cert. and Marriage Certificate are both “NSO” stamped while my son’s Birth Cert. is “PSA” stamped. DFA ASEANA accepted it both, so I am not sure why some of the comments here are confusing (NSO not accepted anymore – maybe because its blurred or a bad copy). LOL anyways, that is just FYI. Kakaapply ko lang ng passport kahapon. I got my BC (NSO) last 2012 pa but DFA accepted it naman. BC, Voter’s ID and NBI clearance lang dala ko. And in less than 30 minutes nakalabas na ako ng DFA office 🙂
Pano kapag di ka pa nagtatrabaho tapos ang valid id mo lang postal at bir pwede kaya yun sa valid ids?

Hi, just want to ask po. Yung akin kasi nung pumunta ako ng dfa. Magpapa “Supplementary place of birth” pa daw ako sa LCR. Hospital lang kasi nakalagy sa place of birth ko, dpat may Quezon City manila daw after ng hospital. Same dn po ba ng process yun and hiningan po ba kayo ng authorization? kasi im planning na kuya ko kkuha for me sa Manila since im working here sa cebu po. Sana mabasa nyo po.
Same case ko sa DFA, Hospital name lang nakalagay sa BC. Friend ko nagaayos ng saken kasi malapit siya sa Manila City Hall. Pinayagan siya ng personel ng LCR Doon. Nagbigay ako sa kanya ng Authorisation letter and ID’s. Patawagin mo brother mo sa LCR ng QC para ma check kung tatanggapin dun. As of now, pinoproblema ko yung sa hospital, kasi mukhang di na existing.

Kasi po yun ID Voters at Registration Record po ng asawa ko ay kulang yun name nya, CHRISTOPHER ISHMAEL po yun real name nya talaga pero yun po kasi nasa Voters ID nya ay CHRISTOPHER lang. Pero sa Birthcertificate po nya at sa TIN ID pati po sa POSTAL ID at NBI Clearance ay correct naman po “CHRISTOPHER ISHMAEL”. Parents ko from davao nag travel sila to gensan to file for passports. Yung dad ko na approved yung mama and kapatid ko hindi kasi daw yung birth certificate nila photocopy lang. eg sabi ng LCR OKAY lang daw mag file gamit yun. may tatak and confirmed by civil registrar and certified true copy yun but DFA wont accept it. On the way pa kasi original nso nila. Does it really have to be original nso? Bat sabi ng LCR okay ang photocopy. Sumakit ulo ko. Nag travel pa sila ng ka layo layo tapos hindi pala sila tumatanggap ng copy of their nso. I need answer please. Thanks.
as far as I know they really need the NSO/PSA original copy of birth certificate
hmm. bakit daw hindi tinanggap yung bc from local civil registrar? nakalagay naman sa website nila (for renewal) psa or local civil registrar copy ng bc pwede. and alam nyo ba if they need a new civil registrar copy din? mine is 2002 pa. i don’t have psa pa kasi. nso lang.. so i was hoping na tanggapin yung. local civil registrar bc.

At ano pong mga importante na details ang tinitingnan nila sa BC na dapat clear, kase may ibang entries sa akin na nagsasapawan na ang words sa line nya sa baba, kaya medyo malabo siyang mabasa pag di malapitan.

Now, I got my updated valid ID and the place of birth is “NCR -Manila”. Hindi siya same ng nasa Birth Cert ko. Nakalagay sa birth certificate ko ay ang pangalan ng ospital.

So, my appointment will be on Feb. 27 but I have a problem with the spelling of my last name in my NSO, instead of an E, naging I. My mom went to the LCR pero sabi nila it will take 6 months for it to be changed. Will NSO consider? Ano po sa tingin niyo dapat kong gawin? May planned family trip kami this April and it would be sad if di ako makasama because I dont have a passport.

I think your details is what matter most. Unless iba yung inunput nyo sa DFA appointment, baka don kayo matanong. Paki kwento po kung ano mangyayari pagpunta nyo.
Yung sakin mali na typo yung nakalagay sa DFA appointment ko na instead of 1993 eh 1992 yung nalagay sa birthdate. magkaproblema po ba yun or pede na dun na iadjust mismo sa DFA yun? hmm.

Hi everyone! may friend akong nag apply ng passport last month at hindi tinanggap yung bago nyang NSO, hindi po luma or decade old yung NSO birth cert nya, pero hindi parin tinanggap, pinakuha siya ulet ng PSA dahil yun na daw ang name ng office at malinaw naman daw na yun ang nakalagay sa website sa list of requirements. First time ko din kumuha ng passport (last January 15, 2018 ang appointment ko) at pinapabalik po ako dahil they were asking for supporting documents kasi ang nadala ko lang po is Postal ID na bago, NBI clearance at PSA copy na birth certificate. As of now, my docs are the following:
Postal ID

PSA copy of birth certificate
Police clearance
Philhealth MDR (they specifically asked for this)
SSS Member data form (since 2 months pa ma release yung UMID po)
Alumni ID
Old college ID
Cedula
They asked for TOR and MDR but ang TOR ko po is 3-4 weeks pa ma release kaya dadalhin ko po yung alumni id tsaka old college id since andun naman sa list. Tapos d rin po ako makakuha nang voter’s certificate kasi d po makita sa comelec ang record ko kahit registered po ako.
Sa tingin nyo po enough na ang mga ito? Thanks po sa advice.

Hi, Mommy Levy! I read Aileen’s comment about the documents. I heard na sobrang strict na daw nowadays. In DFA’s old list, they used to accept the old college and company IDs, premium postal ID for primary identification but now they ask for the UMID, driver’s license etc. A girl who went to DFA Alimall told me that they asked her to reschedule once she has the SSS ID kahit ang dami na niyang dalang documents. Sobrang strict daw dun and she read a lot of negative feedback so she decided to wait for a schedule sa Novaliches na. I plan to get a TIN card, police clearance, SSS MDR in case hindi marelease in time yung ID. March pa appointment ko but I heard the UMID sometimes takes 2-3 months to be sent. I hope Aileen provides a feedback about her experience after. Thanks!

Got the same problem with Ren, wherein the only missing in the place of birth is ‘Manila’. In my BC is just the name of the lying-in, its address and Tondo. So I’m thankful that I was able to read this blog while doing my research. I prepared all the needed documents, even the DFA appointment paper that says I need supplemental for PoB, so I can l just pass it and hoping to be processed that day. However when I came to Manila City Hall they gave me a different requirement list, which is for CORRECTION OF BIRTHPLACE. It got more requirements that includes a certification from the hospital/lying-in of their address. I showed the DFA paper to the personnel at Step1 but mentioned that these are what I need. As I’ve checked they still have the same requirements for supplemental records so I know that they gave me a different list from what you’ve processed with Ren. I’m confused and decided to just go home and think of what to do, since I also don’t know the exact location of the lying-in where I was born. I’m currently working na din po and got a little time to processed these papers.
PS. There is also a registration fee of P1000.00 that is indicated below that made me even believed they got me wrong.
Should I go back there and ask for the supplemental requirements instead? Your suggestion will be a great help po. Thank you in advance.

I’ve got same problem with your son, no ‘Manila City’ in the Place of Birth, just the lying in clinic’s name then Tondo. And I’m thankful I got the chance to read this blog while doing my research. So i’m all prepared with my documents when I went to Manila City hall. However, when I got to the City Hall just this morning they gave me a different requirement. 🙁 They gave me the requirments for the correction of Birth place which includes going to the clinic to secure some certification. 🙁 As i’ve check they still have the same requirments for the supplemental report. I dont know what to follow. I’ve got little time since I’m already working na po.
PS. There’s a fee for registration that is included. I’m thinking I am already registered and its not right to pay for another registration when i just have to add ‘Manila’ in my Place of Birth?

Yes alam ko kailangan pa ng affidavit pag naging problema ito. Pero sa tingin ko mas importante ang details mo kesa sa parents (but I’m not sure since I am not connected with DFA). Perhaps you can inquire directly sa kanila kung magiging problema ba yun sa pagkuha ng passport mo.
Hi, mommy levy! I’m experiencing the same thing too 🙁 I really am desperate because why should I fix something that wasn’t my fault in the first place? The problem with my birth certificate is that the place of my birth is abbreviated. LC ang nakalagay instead of Lucena City. I already filed for a correction but they said it will take up to 4-5 months. Why is that? 🙁 I’ve read that your son was able to get it corrected within 7 days. Is the location a factor? I live here in Quezon Province. Should I go to Manila mismo para mapacorrect ko po agad yung birth certificate?

it took us a month to correct it (PSA copy) not 7 days Melody. Yes I think the location is one of the factor.

Medyo nagworry ako sa question ng isang reader. I checked my husband’s PSA birth certificate and indicated sa place of birth is Cab. City for Cabanatuan City then abbreviated yung Nueva Ecija as N. E. Would that be a problem, Mommy Levy? Kasi if ever, papalakad ko na. Sa March yung appointment namin na buong family. Nakakaloka kasi di naman fault ng mga tao yan. Yung mga nagfill out for NSO before may kagagawan. Tsk tsk.
Would 2 PSA marriage certificates be enough? Kasama namin ng husband ko daughter namin sa pag-apply. I filled out her info sa appointment kasi mag-8 years old na siya next month.
mam kung yung nabasa nyo ay base sa experience ng mga nakapunta na, I think it’s best if ipapaayos nyo na habang may panahon pa. Kesa mag antay kayo ng March tapos di po tatanggapin. I am not connected to DFA so I am not sure if they will accept it or not.

I’m a freelancer and I don’t know how to drive so yung premium postal ID lang gov’t issued ID ko. Ang tagal din kasi kumuha ng ibang IDs. Our appointment is in March 2018. my husband has his driver’s license and premium postal ID along with his old employment IDs kaya wala na siyang problema. For our daughter, we’ll just bring her old school ID, form 137 and birth certificate.
Yung birth certificate ko has corrections. Inayos na namin sa local civil registrar in 2010 dahil sa misspelled name ko, last name ng father ko and other errors. Yung original copy submitted sa LCR tama eh. Yung pinasa sa NSO dati puro mali. Nilakad ng auntie ko in Rizal. I live in Cavite so isipin yung hassle if ako yung pupunta. I just gave my authorization letter and IDs sa school and company to show na yung spelling dun yung gamit ko my whole life. I have NSO copies of our birth certificates and LCR copy of our marriage contract. Same with you hindi ko din magets bakit need ng new (within the year) copy from PSA. Kawawa yung mga pinapabalik tapos galing pa sa malalayong lugar or pinanganak sa provinces.
I think you’re good to go. Di ko din gets bat nagkakamali sa NSO kung tama naman sa local registrar
Sobrang strict nga nila kaya kawawa yung mga may errors sa names and places of birth kahit hindi nila fault.Sa case ng mom before ko pag check sa NSO, Fe yung nakalagay na first name niya. Yun daw ang niregister pero all her life, ang gamit niya Nilda. Inabot ng months bago naayos. Buti may relative kami who works in the municipal hall sa province namin na tumulong. Kaya I checked all our documents. We went to the LCR last year kasi napansin ko yung place of marriage namin sa birth certificate ng daughter namin is Manila City Hall instead of Manila City lang though dun naman located talaga yung LCR where the marriage was registered. Sabi ng LCR, no problem naman daw yun. Kahit hindi na palitan.

I had a passport appointment last Jan.06, pero nung chineck po nila place of birth ko sa PSA birth certificate, walang naka-inidicate kung saang city so, the officer told me to have a certified copy of filing ng supplemental report from the city hall to add the city in my birth certificate record. Itatanong ko lang po sana, kung yung copy lang ba ng supplemental report yung dadalhin ko kapag bumalik ako sa DFA or need ko hintayin na ma-update sa PSA yung pina-add na city sa birthplace ko?

Same situation here. San ka nagapply ng supplemental? When I processed mine sa Manila City Hall they gave me the Correction of birthplace requirments that will also tale 4-5 months to process.

Meron lang din po ako concern regarding din sa passport application. I already have the supporting documents which is comelec certification and NBI clearance. We are waiting for the improved postal ID as the primary documents. Naka schedule na po kasi sa Jan 15. Just incase hindi pa po dumating ang postal ID, mapprocess pa din po kaya un application? Or pede kami humingi ng extension para makabalik ulit without making a new appointment? Wala na po kasi talaga makuha available slots mabilis po maubos.

Hello po, yes nga po. Appointment namen nun Jan 15, we only have NBI and Comelec Cert kasi hindi umabot un postal ID buti na lang po na process naman. Pero I think depende sa processor or san Satellite office yun iba po kasi mahigpit talaga.

wala pong way para mabago ang details na na input nyo kundi pag punta nyo na lang don sa DFA mismo.
Hello po! Tanong ko lang po sana kung kakailanganin pa po bang magpa sched ulit kahit na pinapabalik na lang po ako sa DFA pag na-fix ko na raw yong na-trace po nilang mistakes sa birth certificate ko or dalhin ko pa rin po yong dati kong appointment form? Oct 21,2017 po nong pumunta ako sa DFA. Balik na lang daw po ako dun, Monday-Friday raw open sila pero di sinabi sa akin ang exact date po ng pagbalik. Eh natagalan po sa pag-aayos ng birth certificate ko kaya this Jan po binabalak kong bumalik don.
alam ko may validity time frame yung pag balik pero di ako sure gano katagal. If malapit kayo sa DFA, better kung macheck nyo na lang kung tatanggapin pa nila yung scheduled date nyo. Sabihin nyo na lang na wala naman sinabi sa inyong date.

Mam. Aapply po kase live in partner ko. Mag oonline po kase kami sa passport kase asa aklan po kami tas iloilo papo kukuhain kaya po magpapasched nalang po kami. Yung problem po namin e. Kelangan namin magdala ng isang valid id niya which is he only have a Voters Id card but sadly his date of birth is incorrect . Pwede parin po ba yun magdadala nalang kami ng PSA cert. Almost same kme ng problem ni Ren, mine the place of birth is present however its incomplete, naka lagay lang yung name ng clinic without the specific address.
May passpost nko before kaso need ko magpa renew. Nakausap ko na yung sa Manila City Hall, makukuha ko daw yung finals docs after 4-5mos, then sa PSA after 1-2mos daw.

as of now sa DFA website naglalagay na sila ng warning bago mag input na icheck mabuti ang ilalagay bago isubmit kasi nagko cause ng delay. Maari pong pwedeng palitan sa DFA mismo, pero baka madelay kayo or ipaulit. Hindi ko po alam.

Just want to ask…my wife’s name is mispelled on our certificate of marriage, can she still renew her passport at the DFA? the last time she applied for a passport, we just used an notarized affidavit stating that the person in the birth certificate and the certificate of marriage is one and the same. do you think it will still work?

Written in DFA website:
• Marriage Certificate of minor’s parents duly authenticated by PSA (for legitimate child)
And in this requirement:
• Personal appearance of either parent and valid passport of parents (if minor is a legitimate child)
About the valid passport; does it mean both parents passport, what if the other parent is ofw? Or does the whole sentence mean personal appearance and valid passport of either of the parents?

Yan din problema namin mali yung place of birth sa application form wala kasing nakalagay kung place of birth ba yun or what then nagulat lang kami na yung place of birth ay may nakalagay sa lugar na mali so pwede bang ipabago na lang yun sa step 1 total sa step 1 naman ay verification. Tyaka hindi naman siguro namin kasalanan kung mali yung nakalagay na place of birth kasi wala naman talagang nakalagay na place of birth nagulat na lang kami na may nakalagay at mali?

If you still have time to get a PSA copy then do so, if not then perhaps you could try and just bring your NSO copy.

Yes you have to get PSA copies po. Hindi na po tinatanggap sa DFA yung old NSO Copies. Yun kasi sabi sa PSA office at sa friend ko na kakakuha nya lang din ng passport.

you can get it from your local municipality. It is different from PSA.

if you have read some comments here, sabi nila some DFA station accepts NSO copy pa yung iba naman daw hindi na. If you still have time, go get a PSA copy para sure.

Hi po! I saw your blog because I was searching if there’s anyone who got rescheduled with DFA due to documents or requirements na hindi po tinanggap. As for my case, yung TIN ID and POSTAL ID ko ang hindi nila tinanggap. I just got a question po, in case you would know, kasi the personnel advised me to go back to submit valid IDs and wrote a date above my original appointment date (Nov 3). She wrote po ng Dec 3. I was unable to catch what she said kung “on or before” or kahit after since Dec 3 would fall on a Sunday… Did you had a similar experience po nung pinabalik kayo to submit the “valid” requirements po? Question po, ano po ba yung kulay ng nso birth certificate ang tinutukoy dito?white or yellow?
Parehas kase ako meon ng nso birth certificate, white and yellow. Then yung brother ko nag apply ng passport using yellow na nso birth certificate na tinanggap po sa dfa and naprocess naman po passport nya.
mam logo lang po ang dapat pansinin NSO or PSA copy. PSA copy na daw po ang tinatanggap ngayon.
Hi po. Katapos ko lang po mag apply and tinanggap naman po nila yung nso logo na yellow colored na birth certificate ko. Mabilis lang po wala pa 30mins naprocess na agad. 🙂

Galing kami ng husband ko yesterday sa dfa. And nakita na walang gender sa psa birth cert nya. Pero ng chineck ko sa issued ng city hall na birth cert meron nman. The problem is sa isabela pa xa. Kailangan ba tlga nya pmunta pa run para magfile ng suoplemental report. Thanks

Wala rin po sinabi saking date kung hanggang kelan ako pwede magpasa. Nagkaproblema din ako sa requirements. Last Dec 14 yung appointment ko eh. Babalik ako bukas Dec 19 sana pwede pa 🙁

check your email, you should have the PDF copy of your appointment schedule. I don’t know if they will still accept NSO copy. If you still have time better to get the latest copy (PSA) to be sure.
Hi, i think NSO is the same as PSA.. New name lang ung PSA ngaun coz when I check their website it shows there formerly known as NSO. Thank.
yes they are the same, pero ang pinagtatalunan po kasi is kung tatanggapin ng DFA yung lumang papel na NSO ang nakatatak. May ibang area kasi na tinatanggap may iba naman na gusto latest copy na.

Just want to ask what did you do next upon getting the supplemental report? Did you get another copy of PSA BC reflecting the missing information?
I applied for a supplemental report just today since the clinic where I was born has no specific city in it. The officer in our city hall did not mention about its transmittal to NSO, but I paid 150 pesos. Close enough to 190 pesos you paid.

I was the one who brought it to NSO and I get another copy with the corrected details on it.

Oh I see. Sorry to bother you Ms. Levy but I have one last question. Hope you can still accomodate. When you mentioned that you went to NSO QC office, is it at PSA-CVEA Building, PSA Compound, East Avenue, Quezon City, to be exact? I’m overwhelmed with many office locations published in google.

you mean the existing NSO copy is blurred? If it is not readable at all, then I suggest to apply for a supplemental report in your municipality of birth the bring it to NSO for correction. When you have the corrected copy that’s the time you can bring it to DFA.

Hi blurd birth certificate PSA kalaunan Lang but blurd nga.. Meron din copy ng local civil registrar kelangan pa ba ng supplemental report dahil blurd ang nasa PSA birth certificate . Or ok na yun sa dfa .. Birth certificate na blurs from PsA at local civil registrar na malinaw pero old copy naitago lang.. Need pa ba ng supplemental then after nun ibabalik sa nso para maging malinaw…
get an updated copy of local civil registar together with your latest copy of PSA. If wala talaga mabasa yes you need a supplemental report po.

Hi. I was at Dfa Sm manila yesterday and I submitted my PSA BC. I WAS BORN OCT 3,1991 and as we all know medyo madumi naman talaga PSA copy. So my problem is, yung 3 kasi parang may kudlit na manipis na manipis kaya nagmukhang 31, but kung babasahin sa ibaba pumirma yung mom ko Oct 4, so how come sasabihin nilang 31? So sabi nya kelangan daw pumunta sa local registrar at dun malinaw na malinaw naman walang kudlit. Anu kaya pwedeng gawin ma’am?
I know this is frustrating but we need to follow their request and instruction. Just visit the City Hall of Manila and get a local registry copy. Did they ask you to file a supplemental report? or pwede ng dalhin yung local registry copy mismo?

I’m planning po pumunta sa Manila city hall kasi like you po hindi rin tinanggap ng dfa yung nso copy ko kasi name lang ng hospital ang nandun at walang “Manila”. Supplemental form din po kaya kailangan ko? And sa tingin nyo po, ilang months aabutin? Sa pagkakaalam ko kasi name of hospital lang talaga meron at ang kailangan nilam ay at least “manila” daw po. Although Govt issued po ang mga papel natin at walang nakasaad na expiration date, it is really a must to have the latest copy. Because govt systems are being maintained and needs to be upgraded regularly. As such, some of its data may have been altered because of our negligence. Example is, double registration between different municipalities.

As clearly stated on DFA website, you only need to present LCR 1A if there are inputs on your PSA copy that are unreadable or blurred.

Fyi… I was able to get my passport without being asked… the discrepancy doesnt matter to them or they dont even look at it.

I have the same problem with your Ren ,my birthplace address is missing,name lang ng hospital ang nakasulat. so yung remarks sa DFA application ko po ai “to submmit annotated PSO copy regarding birthplace.” I already filed supplemental blah blah in our municipality. they’ve told me to wait 3 mos. eh baka matagalan pa daw kung hndi ko i fo follow up sa NSO manila mismo .From Baguio pa naman ako .Is there a faster way for me to get my passport. I think you need a supplemental report for that especially if gamit mo is Torres na middle name.

Hi I went to DFA for my babygirl, and it was registered late but I have gotten the true copy from Local Registrar, and when I went to DFA they told me I had to go to NSO to get it certified true copy “stamp” what and how much does that cost ? The hospital messed up registering her and it was late and was not registered till JUNE and she was born JAN, what do I need to ask for NSO to get my babys passport because we have to travel OCT
You need to wait for the NSO/PSA copy. That document is just what DFA needs, nothing else as far as I know.
Hi Ms. Jasmine, you can go to PSA main office at sta.mesa and have it stamped as certified true copy for 140.00 pesos. That will suffice the requirements eve if the NSO copy is not yet available.

I think correction po kasi pareho ang local at PSA. Yung sa’min kasi tama ang local mali sa PSA.

as far as I know, need talaga correction. Hindi ko alam pinagkaiba ng affidavit sa supplemental report. Yung supplemental report inabot ako 1 month para matapos at maayos.

I am not sure, pero sa site nila nakalagay naman sa site nila na Marriage Contract (MC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA.

Lagi lng pong Certified Machine copy yung bnbgay sakin. Yung parang photocopy lng ng BC tpos may seal/tatak na violet. Tntnggap po ba yun?

I think pwede dito ang supplemental report. Just visit the town kung san pinanganak mama mo then ask for a supplemental form and complete the requirements and just follow the procedure.
If only there is a pre-evaluation process where we can just take a picture of our documents, send it to them via email (or perhaps an additional step on their online application) for pre-evaluation. When approved, that will just be the only time that we go and bring the original hard copies of the requirements. Imagine if this has been implemented in any government agency, perhaps we can even lessen the traffic. Don’t you think?

I have the same issue. Place of birth written in my bc is the name of the hospital and dfa asked me to file a supplemental report for it to be changed to or to add “Manila”. You said it only took you a month to process pero saken 3months daw. Hays…

Hi mommy ask ko lang kung kailangan talaga ng marriage cert pag inapply ko ung baby ko ng passport nya, naka book na kame ng July 08. Ang problema po kasi is ung name ng daddy nya sa MC mali ung format. It should be Ching-Wei Yi po pero don sa MC Yi Ching Wei. Kung need po talaga ng MC tanggapin kaya nila ito kahit na ang last name ng baby ko is Yi na correct naman po don sa birth cert nya. Thanks

Hello, I believe supplemental only applies to missing information sa PSA copy ng BC mo. If meron naman nakalagay pero kulang or mali, they will require correction.

Hi galing kami dfa pampanga my dala na kami affidavit of discrepancy para sa name ni mother. Tinanggap naman ng dfa. And need nga 2 id though isa lan nadala ni mudra eh punagbigyan n cya. Hindi na din allowed na makakuha ka din ng passport kahit senior kasama mo. So i had no choice kundi i renew na yung s anak ko na 2 years ago p lang passport kesa byahe kami ulit. Pag anak allowed both parents pag kasal. Pag daw nde eh nanay lang. Pag renew pala ang need na lang eh photocopy ng mga id at old passport. Change Surname ako kaya need ng marriage cert. sulit ang luwas dahil nakakuha kami 3 naman. Share lang po. 🙂

Hello po. Bukas na po yung schedule ng mama ko sa dfa. Pero ngayon lang po napansin na may not readable sa bc niya. May nakalkal po kaming lcr form 1a niya pero issued po siya nung 1997 pa. Valid po kaya yon? Hindi na po kasi namin maprocess ngayon. And kung hindi po tatanggapin yon, pwede po kaya na ang isubmit na lang ay yung original bc niya? As in yung green po na bc. 😣
hindi po nila tatanggapin ang original bc na green, NSO copy lang tinatanggap nila. Try nyo po muna since bukas na din naman yun schedule.

Good eve po! Naka-sched po kasi ang passport appointment ko this Saturday sa DFA-Alimall. Suddenly, as I browsed through my necessary requirements (just to prepare it for Sat), I found out that my NSO BC is not the updated one. Then I realized po na last 2007 pa po ako kumuha ng copy ng BC sa NSO. Ask ko lang po if do I really need to get a latest copy of my BC? Kasi I suppose na readable pa naman at mukhang bago yung copy ko ng BC. I do not have time na po kasi to get another copy due to my hectic sched, i.e. may pasok po ako during office hours everyday. So I’m contemplating on the validity of my BC. Thanks for immediate response in advance and God bless!

Again, unexpected issue. DFA SM Manila, they did not accept my NSO Birth certificate because the year of birth was not readable. They need a supplemental report from the city of birth’s civil registry office as proof of the date of birth. Is this our fault if NSO does not give a clear copy of the birth certificate, OR if theres a part of it that is not clear right there and then thay advise us to acquire a certificate from the civil registrar since the birth certificate will not serve its purpose having the said unreadable info. S@#€!
I agree, this is just unfair because in the first place it’s not our fault. This correction takes time, energy and money. Hindi biro.

Hi po, nagkaissue din ako sa birth certificate ko due to place of birth, instead of Quezon city and nakalagay po eh NLIC (Novalichez Lying In Center) so I need supplementary report achuchu din na nakakahaggard plus need ko pa ng TOR (March 17 ung sched ko) then kakakuha ko lang ng TOR yesterday and ung birth cert ko eh kukunin ko pa lang bukas busy kasi eh pero nagwoworry ako na baka need ko ulet magpa appointment kasi it’s been 2 months na? Hi magbibigay lang po aqo ng update. I went to DFA Alimall last 29 of May and I was declined nga kasi my appointment is already expired, totoo nga na 30 days lang ung validity ng appointment but the good thing is, may appointment ako na bago and that was yesterday. Every thing went smooth naman and I was able to use my old college ID (since I don’t have any other valid IDs) to think na it was given to me 10 years ago (malaki na kasi nabago sa ichura ko). The good DFA guy (bless his heart) only asked me for TOR, good moral character, birth certificate with supplemental report, NBI, police clearance, photocopies of my other IDs such as Solo Parent, Postal (not digitized), PhilHealth and company ID and viola! It only took me 45 mind to pay for it and have my picture taken. I’ll be getting my passport on June 27 at the very long last! I was able to move my HK trip on the first week of July kaya wala na qong problems, phew! Thanks po sa pagtulong!

As far as I know they will follow what’s on your birth certificate. If malayo pa ang sched mo sa DFA, better to visit them and ask if your BC is okay or not so that you can get a supplemental report as soon as possible.

I think what DFA means is you get a supplemental report. They will not accept hospital certificate even Baptismal, Communion and Form137 documents as far as I know.

Once you are there, ask mo p rin kung ano dapat mo gawin para sure. I went to a whole lot of producing all neccessary documents para lang ung nakalihan kong nameang magamit ko kc from elementary hanggang s nagwowork aq un ginagamit q n spelling until I applied formy passport n maraming maling spelling s name q.

Hi po. Nakaschedule po ako for renewal ng passport sa June 7. Ask ko lang po if kelangan pa talaga ng latest secpa ng birth certificate. The one I have kasi po is the old one (color blue) pero naka red ribbon na. Just want to make sure para if ever di pwede makarequest na ako agad ng bagong nso copy.

First time ko po kasi magrerenew dito sa Pinas and last time na narenew passport ko was on 2009 and expired on 2014 Anyway nakalagay sa birth certificate ko is Certified True Copy of NSO Birth Certificate.

Nagpa schedule po ako ng appointment for new passport application for my wife and my daughter.. 1 copy lang po ang NSO marriage cert ang meron kame. Ok lang po ba un or kelangan 2pcs din po ang Marriage cert na kailangan.. Kinukuha po ba nila ang BC at MC or un xerox copy lang..

Ang alam ko dapat tag-isa sila ng marriage certificate. Kinukuha nila ang original copy, hindi pwede ang photocopy.

Ma’am paano po kapag nagpa’appointment po kami ng Family ko sa DFA noong February 20, tapos yung pamangkin ko lang ang hindi nakunan ng picture kasi Incomplete Documents siya, need pa niya ng DSWD Clearance, Power of Attorney at Consent kasi nasa Japan mother niya. Ngayon lang po namin na’kumpleto lahat ng Requirements. Kailangan po ba magpa-reschedule ulit or kahit dumerecho na sa DFA? Salamat po.
we were informed nung kami na hindi na namin kailangan magpa schedule pagbalik namin. But my son has a PWD ID din kasi, so I am not sure if same sa inyo. As of now, July na ang available slots sa DFA website.

some readers here said that DFA still accept the old NSO copy of their BC. I suggest try it na lang or better yet para sure get a latest copy na before you visit DFA, it’s only P170 in SM and you just need to wait 1 week for it.

My cousin just found out na wala X sa gender nya sa BC. the DFA officer ask her to get a supplemental agreement. Is the agreement enough since wala naman sinabi yung officer na ipa-authenticate sa NSO, o kailangan pa tlga ipa-authenticate which according to your post will take a month. We already have a booked ticket abroad at May 25.
based on my experience DFA need an authenticated/corrected copy of the birth certificate. But, to be sure you can ask them also if they will accept the supplemental report from your local municipality na since malapit na flight.

I’m scheduled for an group appointment to renew my green passport on Jan 17, 2017. I requested (after I made an appointment with DFA Alabang) a copy of my Birth Certificate last December 2016 and found out there were errors on my certificate . Now I have to go to my Local registry on January 16 to fix it and it would take one and half months to process, What will happen when I go to DFA Alabang on Jan 17 with incomplete requirements specially the PSA Birth Certificate. Would they still entertain me and process my application and just ask me to submit the need requirement or ask me to set other appointment and return when I have completed my requirements? My family is planning to get a group appointment for new application of passport. My son is only 5 years old. I am preparing the needed documents and I’m wondering if DFA will get the original marriage certificate for my son’s supporting document. Because if so, I will need to get at least 2 original PSA marriage certificates for me and my son’s passport application. Am I correct?
2. Is NBI Clearance a REQUIRED supporting document?
I checked the list of supporting documents online and mentioned in the site that I need to bring at least 2 or 3 of the listed docs. listed were marriage contract, land title, transcript of records, nbi, brgy clearance, digitized postal id etc.
I already have 3: marriage contract, transcript of records and digitized postal id. Do i still need to acquire NBI Clearance in addition or as part of the 3?

Hi, halos same scenario kaibahan lang no name yung sa NSO BC ko. I was applying for a passport. Sabi ng DFA punta daw ako sa cityhall kung saan ako ipinanganak. Sa remarks ng application ko sa DFA supplemental report reg. first name. pero sa CTC ng BC ko andun naman yung first name ko. pero still they prefer NSO copy. galing ako ng manila cityhall kanina, balik daw ako ng Jan 05, ano ba yung makukuha ko on Jan 5? ayun na ba yung supplemental report ? pero sabi sa dfa aabutin daw ng 3 wks proseso nun?

Iba pa ung supplemental report sa certificate of on going process no? So same kasi kami ng anak mo, however, ako yung need ng passport. So ano requirements ni supplemental? Di nako nagbackread kasi iba iba questions nila.

And when I went back to DFA Alabang they did not issue my passport for they were asking a “Petition Granted” paper something like that. I’d like to ask did they issue your passport? What did you do? Pano po kaya yun nag fill up po ako ng Appointment Online para sa renewal ng passport kaya lang po hindi ko napansin na hindi ko po pala nalagyan ng Middle Name pero dun sa old passport ko may Middle Name po ako…Ok lang po ba yun or may conflict? Salamat Po!
